The National Diabetes Prevention Program

The National Diabetes Prevention Program (DPP) is a 12-month program and will start in October. Each week, a new session will be available online for the participant to view. If you are considering joining our Diabetes Prevention Program (DPP), please view the National Diabetes Prevention Program (NDPP) on-boarding program video (4 minutes):
To qualify, candidates must complete the online health screening assessment by September 24, 2021.This is a private
1-minute online assessment to gauge your risk of pre-diabetes. If your online assessment comes up positive for potential pre-diabetes, you will be eligible for the program.Online private assessment link:
A provider will contact each individual that qualifies and will discuss the program details and objectives.
Program will begin the week of October 4, 2021.
Type2 Diabetes is a terrible disease that can be prevented if you take action before it’s too late!
Take steps now before it affects you and your family. A majority of the participants in this program succeed
and reduce their blood-work levels to normal levels.
The National Diabetes Prevention Program is covered by most insurances.
