Request for Medical Records at The Queen’s Medical Center

All requests for medical information must be made in writing to Queen’s Medical Records Department, Release of Information Section. Medical Records does not maintain or release radiology films or billing records.

Please download the Authorization to Release Medical Information form, print and complete. Forms must be signed by the patient, legal representative or legal guardian.

Fees apply and must be paid in advance.  There is no charge for medical records directly sent to a physician or health care facility.
