Diabetes Management & Education

The Queen’s Medical Center provides outpatient diabetes education in a comprehensive, interactive approach that teaches how to successfully self-manage your diabetes and develop health practices that fit into your life.

Queen’s diabetes programs are recognized by the American Diabetes Association as meeting the National Standards for Diabetes Self-Management Education.

About Us

The Queen’s Diabetes Management and Education Center provides outpatient diabetes education in a comprehensive, interactive approach that teaches how to successfully self-manage your diabetes and develop health practices that fit into your life. Your care team of skilled health professionals and certified diabetes educators will collaborate with you and your physician(s) to help better manage your diabetes. 

The program provides diabetes self-management education and support to persons with type 1 and type 2 diabetes.

Our Services

Diabetes Self-Management Education (DSME)

  • Recommended for all people with diabetes, whether newly diagnosed or needing new strategies for successful diabetes management
  • Certified diabetes educators teach you the skills needed to manage both type 1 and type 2 diabetes 
  • Provides comprehensive education on the many aspects of diabetes care and lifestyle management, including:
    • Goal setting
    • Nutrition counseling and meal planning
    • An active lifestyle
    • Oral and injectable medications
    • Blood glucose monitoring
    • Reducing risks and complications
    • Healthy coping
    • Disease process
    • Problem-solving
  • Both individual or group sessions are offered
  • Our program is interactive, with time provided to practice the skills and tools needed to improve diabetes self-management
  • Ongoing support is provided through follow up visits, phone calls and email support

Medical Management

  • Nurse practitioners work with you and your primary care provider to start and adjust diabetes medication so you can maintain blood sugar balance
  • Nurse practitioners specializing in diabetes care recommend or prescribe the latest technologies for diabetes medication and monitoring
  • Services are provided in partnership with your primary care provider, obstetrician, or endocrinologist
  • Your treatment will be personalized to address your medical conditions, schedule, activity level and lifestyle

Medical Nutrition Therapy (MNT)

  • A registered dietitian will help you integrate diabetes nutrition and self-management skills into your lifestyle to enhance your diabetes management
  • Your sessions are personalized to your medical conditions, schedule, activity level, and lifestyle
  • Education is provided by a registered dietitian with experience in diabetes care and education who will coordinate your nutrition care with your diabetes care provider
  • Appointments are also available for nutrition and healthy eating plans for other medical conditions

Becoming a Patient

The Queen’s Medical Center provides outpatient diabetes education programs with an interactive approach that teaches you how to successfully self-manage your diabetes and develop health practices that fit into your everyday life.

Managing Your Diabetes for Life

Managing Your Diabetes for Life is an education program that explains how diabetes develops,  teaches you how to self-manage your condition and prevent complications. This is a four (4) class series and requires a one-on-one initial appointment and screening before you are eligible to enroll.

Classes are held monthly, every Thursday, from 9am – 12pm. Located in the Queen’s Physicians Office Building 2, Suite B-5.

  • Cost may be covered by most insurance plans, please check with your specific provider
  • A physician referral is required before the one-on-one initial appointment and screening

To refer your patient to the Diabetes Management and Education Center, initiate an electronic referral via Epic (CARE*Link) or fax the forms below to our office at 808-691-5399. 

Diabetes Self-Management Education (DSME)

Includes individual or group education and option for Diabetes Medication Management by APRN-Rx, CDE. MD or APRN referral required. 

Medical Nutrition Therapy (MNT)

Includes dietary education with a Registered Dietitian for diabetes, chronic kidney disease and most chronic conditions, depending on insurance coverage. MD referral required. We suggest that patients being referred for obesity who have large amounts of weight to lose (BMI of 35 or more) consider referring to Queen’s Comprehensive Weight Management Center for multidisciplinary, non-surgical and/or surgical weight loss approaches.

Diabetes In Pregnancy Program (DIPP)

Includes education and support for women with gestational diabetes or pregestational type 1 or type 2 diabetes. MD or APRN referral required.
