Continuing Care

The Continuing Care department (also known as Social Work) at The Queen’s Medical Center - West Oahu assists patients and families in identifying psychological issues related to hospitalization and provide counseling services for critical decision making for care and adjustment to illness. We also help with complex discharge planning needs as well as coping with issues after discharge from the hospital. Our licensed social workers are trained to help you through the challenging course of your medical journey. We work with patients, their families and the health care team to coordinate and facilitate treatment throughout the hospital stay until discharge.

Our Team

Our Continuing Care team of social workers are Master’s Degree trained, culturally sensitive, and follow the National Association of Social Workers Code of Ethics. The caring staff are credentialed as Licensed Social Workers or Licensed Clinical Social Workers and are experts in areas including crisis intervention, pain and palliative care, and trauma. We believe advocating for the needs of our patients and their families generates stronger communities.

Master of Social Work (MSW) Graduate Training Program

The Queen’s - West Oahu Continuing Care Department provides training and supervision for the University of Hawaii, School of Social Work and Hawaii Pacific University's Master's degree candidates, with the MSW Graduate Practicum Training Program. Second-year students work under the direction of a social work instructor and train at our facility for a minimum of one year. An average of 10 students are selected annually for the program.
