Diabetes Self-Management Education and Support

We help individuals who are newly diagnosed with diabetes and those who are in need of new strategies to successfully manage their existing condition. We offer:

  • Private and group education sessions based on how you learn best
  • Interactive learning so you can practice the new skills and tools you’ve learned
  • Support after program completion continues through office visits and phone calls

Medical Management

Nurse practitioners specializing in diabetes care will work with you and your primary care provider, obstetrician or endocrinologist to start and adjust medication and monitoring. Your treatment will be personalized to address your medical conditions, schedule, activity level and lifestyle.

Medical Nutrition Therapy and Dietary Counseling

A registered dietitian with experience in diabetes care and education will help you integrate nutrition and diabetes self-management skills into your daily routine. Appointments are available for nutrition and healthy eating plans to address other conditions.

Diabetes in Pregnancy Program

A diabetes nurse educator or registered dietitian assists women with pre-existing diabetes or gestational diabetes, helping them learn about glucose monitoring, healthy eating and diabetes care during pregnancy. Ongoing support is provided throughout pregnancy.
