The Laboratory at Molokai General Hospital provides testing of cultures and tissue samples to determine diagnoses and appropriate treatments for patients.

Veri-5 Kits

The Veri-5 Kit is a personal drug screen collection kit that can be used confidentially in your own home. This kit can be used as a personal drug test or to test a family member. The five most common drugs tested are: amphetamines (crystal meth), cocaine, marijuana, opiates and PCP.

Urine is collected into the provided container with a confidential PIN number and dropped off at the laboratory. Results can be obtained by phone using the provided PIN number.

A physician’s request is not required for the Veri-5.

*NOTE: Veri-5 results are not intended for medical, workplace or law enforcement purposes

Payment for Services

For patients with no insurance, the Laboratory will give a 50% discount if payment is made at the time of service. The discount can be only offered if laboratory personnel is notified at the time of service.

Cash or check payments are accepted.

For Physicians

A Physician’s order required for blood draw, testing and processing.

Out of state requests will be honored only with the following required information:

  • Written requisition including Physician’s name, address and phone number
  • Test(s) requested
  • Diagnosis
  • Any special instructions
