Health Insurance Coverage

Your health is important to us and we want to provide you with the information you need to make decisions. During this critical time, you may be wondering what to do if you or a family member needs health care.

The Queen’s Health System accepts most insurance types. Your doctor will determine which tests, procedures, and consultations you may need to address your healthcare needs. However, it’s important to note that whether your insurance plan covers your care depends on your insurance provider. Please contact your insurance company to verify eligibility.

If you are recently uninsured or are at risk of becoming uninsured, here are some avenues to explore.

Affordable Care Act (ACA) plans for individuals and families are available through the federal exchange, HealthCare.gov. You may be able to get a government subsidy to help pay for your monthly membership rate.

You may be eligible to get a health plan at no cost or low cost from the state during these difficult economic times. Learn how to apply by visiting the QUEST Integration website: https://medical.mybenefits.hawaii.gov.

The Explanation of Benefits (EOB) is a statement that shows a detailed listing of how your insurance company processed your claim. An EOB is mailed by your insurance company directly to you.

You may be eligible for coverage as a dependent through your spouse or partner’s employer, or another organization. If you are under 26, your parents may be able to add you to their plan.

The following organizations can provide assistance in navigating your health insurance options:

Applying for Financial Assistance

The Queen’s Health System is committed to providing medical services to patients and objectively considers each patient’s financial ability to pay for the cost of hospital services.

Our Financial Assistance Policy

The Financial Assistance Policy offers emergency and other medically necessary services at low or no cost to the patient. A discounted rate is applied for eligible patients who are uninsured or underinsured and do not have or cannot obtain adequate financial resources to pay for their medical care.

Our staff will assist individuals, free of charge, in applying for eligibility government health insurance programs, such as Medicare and Quest. All applicants will be initially screened for Quest eligibility by our partner ElevatePFS and requested to cooperate with our staff in applying for the Quest program. Upon approval of the financial assistance application, individuals will receive either a 100% or a 76% discount.

Eligibility Requirements

Financial assistance is generally determined by a sliding scale of total household income based on Federal Poverty Guidelines for US citizens and US residents or based upon the country minimum wage annualized for non-US citizens and non-US residents.

Financial Assistance Application & Documentation

The following link will allow you to complete the Financial Assistance Application online. Once completed you will be prompted to attach copies of the required documentation to support your application.

Please take a few moments to have all the information needed to complete the application and attach the required documents.

Alternatively, you can fill out the PDF version of the application and email it along with copies of the required documents to ElevatePFS@queens.org.

You can also mail your Financial Assistance Application or visit your local Queen’s medical facility registration or financial services department to turn in your Application.

For Financial Assistance Inquiries:

Call (808) 691-4210 or visit our Financial Services Office.

If you have any difficulty in completing the application or attaching the documents, please contact our partner ElevatePFS at 808-566-6666, who will be happy to assist you. They will also assist in interviewing you for the Quest program.
