Cardiology services are available at two locations on campus. 

To Refer a Patient to the Cardiology Clinic

Our Cardiology Clinic is located in the Physicians Office Building (POB) West O‘ahu.

1. Completed Referral Form – Click Here to Download Form
2. Patient Demographic Information Sheet: Home address, contact information, insurance information, a copy of picture IDs and insurance cards
3. Referring provider’s last (2) office visit notes
4. Recently completed lab results and reports
5. Cardiac imaging reports (echo/treadmill), if applicable
6. Most recent EKG, if available

To Refer a Patient for Non-invasive Testing

The cardiac non-invasive testing facility is located in on level 1 of The Queen’s Medical Center – West O’ahu.
