Geriatric Services

As the largest and most comprehensive geriatric program in Hawai‘i, our goal is to help seniors age gracefully. Getting older doesn’t necessarily mean quality of life has to suffer. We work with seniors and their families to ensure they stay as active as possible, so they can continue to enjoy doing the things they love. From our ‘ohana to yours – aloha.

Our Geriatric Services

We address the full range of conditions that affect the elderly, including the homebound and frail elders. Some common conditions and problems we treat include, but are not limited to:

  • Fall risk
  • Depression and anxiety

  • Unintentional weight loss
  • Memory loss and confusion
  • All types of dementia
  • Parkinson’s Disease
  • Frailty
  • End-of-life issues
  • Medication management
  • Mobility
  • Caregiver education and support

Our services include:

  • Outpatient clinic: comprehensive consultations to frail elders
  • Hospital consultation service: evaluation of older adults who are hospitalized with complex needs
  • Post-acute and long-term care team: continuity of care for patients in partner nursing homes
  • Geriatric home-based primary care: medical management of homebound patients with a team of doctors and nurse practitioners

Our geriatric specialists are fellowship trained. Our team of doctors, nurse practitioners, medical assistants and social workers are compassionate, provide thorough care and evaluations and, most importantly, they listen to you.
