What is MyChart for Teens?

MyChart for Teens is an online tool for patients 14 to 17 years old that provides convenient access to certain medical information.

MyChart for Teens provides limited access to parents/legal guardians to comply with Hawai‘i state laws about health information and privacy for teenagers. Hawai‘i state law allows teens to receive specific medical care confidentially.

What confidential services can I receive?

A 14 to 17-year-old can make confidential appointments with their health care provider for:

How do I sign up for MyChart for Teens?

Account sign-up and activation should only occur in person at a Queen’s clinic or facility to verify that the teenager is the one creating their username and password. The activation code can be sent to the teen’s personal cellular phone or personal email. Staff can assist the teen in activating their account before they leave the clinic or are discharged from the hospital.

What can I do in MyChart for Teens?

For a 14 to 17-year-olds

Teens can also use the Queen’s MyChart patient portal to send private messages to their provider.

For a Parent/Legal Guardian

Parents or legal guardians who want to sign up for proxy access must complete the Child/Teen Proxy Request Form (PDF). If a parent or legal guardian already has proxy access for a child who is younger than 14 years old, they do not need to resubmit the form for access when their child turns 14 years old.
