Follow the steps below to find a cardiologist and become a patient.

Referral Needed for Heart Care

You must have a referral from your Primary Care Physician (PCP) or another medical professional overseeing your care before we can schedule an appointment for diagnostic testing or with a member of our cardiac care team.

Please have your PCP or medical professional overseeing your care contact our office at 808-691-8900.

They will be required to provide your medical records to our offices for review. A full list of required records is available on our page For Physicians.

Once your records are received, they will be reviewed by our cardiac care team. We will contact you to schedule your appointment using the phone numbers provided by your PCP or other medical professional.
