Send a Referral

To refer your patient to one of our board certified, professionally trained physicians, please provide the following records for review:

1. Queen’s Heart Referral Form
2. Patient Demographic Information Sheet: Home address, contact information, insurance information, a copy of picture IDs and insurance cards
3. Referring provider’s last (2) office visit notes
4. Recently completed lab results and reports
5. Cardiac imaging reports (echo/treadmill), if applicable
6. Most recent EKG, if available

If you refer to Queen’s Heart Institute, you can expect:

• Upon receipt of your referral, your patient can expect to hear from our team within 3-4 business days.
