Liver Transplant Program 

Physicians may refer their patients with liver disease that may require a transplant to one of our transplant hepatologists for an initial consultation by calling Physician’s Exchange 808-524-2575 (local) or 800-360-2575 (toll free).

Adult Kidney Transplant Program

Referrals to the Queen’s Transplant Center may be made by filling out the Kidney Transplant Referral Form. The patient should complete and sign the Patient Health Questionnaire for Kidney Transplant Evaluation. Fax forms directly to the Queen’s Transplant Center at 808-691-8896.
