How to Refer a Patient to Queen's Heart Physician Practice

To refer your patient to one of our board-certified, professionally trained physicians, please provide the following records for review:

  1. Queen's Heart Physician Practice Referral Form
  2. Patient Demographic Information Sheet: Home address, contact information, insurance information, a copy of picture IDs and insurance cards
  3. Referring provider's last (2) office visit notes
  4. Recently completed lab results and reports
  5. Cardiac imaging reports (echo/treadmill), if applicable
  6. Most recent EKG, if available

How to Refer a Patient to Queen's Heart Cardiovascular Interventional & Surgical Services

To refer your patient for cardiovascular interventional or surgical services, please submit:

  1. Last two progress notes from the referring provider or Primary Care Physician (PCP)
  2. Images on disc of the patient's most recent transthoracic and/or transesophageal echocardiogram, cardiac catheterization and CT images
  3. Written reports of diagnostic examples, including previous open heart surgery reports, echocardiograms, cardiac catheterizations, CT scans, stress tests, pulmonary function tests, and carotid or arterial duplex studies
  4. Recent laboratory studies

  • Aortic stenosis (Transcatheter Aortic Valve Replacement-TAVR, Balloon Aortic Valvuloplasty-BAV)

Coronary Artery Disease

  • Percutaneuous Coronary Intervention (PCI)
  • Chronic Total Occlusion (CTO)
  • Cardiac Artery Bypass Grafting (CABG)

Mitral Valve Disease

  • Mitral regurgitation/insufficiency (Transcatheter Mitral Valve Repair (TMVR)- MitraClip Procedure)
  • Mitral stenosis (Balloon Mitral Valvuloplasty-BMV)


Structural Heart Diseases

  • Patent Foramen Ovale (PFO)
  • Atrial Septal Defect (ASD)
  • Perivalvular Leak
  • Left Atrial Appendage Occlusion

Thoracic Disease

  • Aortic Aneurysms/Aortic Dissections
  • Thoracic Endovascular Aortic Repair (TEVAR/EVAR)
