How to Refer a Patient to the Queen’s Heart Institute

We truly appreciate your confidence in referring your patients to our care. We want to ensure a warm, seamless handoff as well as necessary follow-up and open communication. With your assistance, our cardiovascular specialists want to ensure your patients receive the highest quality and most compassionate care available. Our team stands ready to confer on available treatments and explore ways to reduce chronic conditions.


Referral Instructions

For outside providers

  1. Please download or print our Queen’s Heart Referral form

  2. Send completed form along with requested medical documentation outlined on form to our fax line or email.
  3. Please call (808) 691-8900 if you have additional questions or concerns with how to submit a referral.

For internal referrals

  • Referrals to the Queen’s Heart Institute can be ordered through CareLink (EPIC)
    • Via order entry, select “Referral to cardiology”
    • Please select internal referral and the department you would like to refer to. (E.g. Queen’s Heart Physician Practice for General Cardiology, location QMC punchbowl)
    • If you have a preferred provider in mind, please indicate in the referral order
    • Please state an indication for referral.
  • Our office will contact you if any additional documentation is needed outside of CareLink (EPIC) records.

For patient-self referrals

Please have the medical professional overseeing your care submit a referral request.

  • Upon receipt of your referral, your patient can expect to hear from our team within 5 business days.
  • If your patient has not heard from Queen’s Heart Institute within a week, please recommend the patient contact us at one of the telephone numbers below:

Mitral Valve Disease

  • Mitral regurgitation/insufficiency (Transcatheter Mitral Valve Repair (TMVR)- MitraClip Procedure)
  • Mitral stenosis (Balloon Mitral Valvuloplasty-BMV)

 

Structural Heart Diseases

  • Patent Foramen Ovale (PFO)
  • Atrial Septal Defect (ASD)
  • Perivalvular Leak
  • Left Atrial Appendage Occlusion

Thoracic Disease
