Complete our referral form: DOWNLOAD FORM (Suite 107) or DOWNLOAD FORM (Suite 704)
Provide a complete demographic sheet with insurance information
Provide the latest progress notes with any relevant laboratory results
Fax all documents to 808-691-5250 (Suite 107) or 808-691-8716 (Suite 704)
Authorization will be procured, if applicable
Once authorization is approved your patient will be called to schedule a new appointment
