Initial Queen’s Cancer Center Consultation

Thank you for choosing Queen’s for the care of your patient. A referral is required for the Queen’s Cancer Center.

Required Records

  • Referral Letter/Authorization
  • Patient Demographic Information (home address, phone number, insurance)
  • Referring Physician History and Physical (H&P)
  • Pathology Reports
  • Pathology Slides (mail to Queen’s Cancer Center)
  • Operative Reports
  • Procedure Reports (EGD, ERCP, EUS)
  • Radiology/Oncology Reports
  • Imaging Reports
  • Hard or Digital Copies of Images (if not done at The Queen’s Medical Center) CT, MRI, X-ray, Ultrasound, SESTAMIBI, mail to Queen’s Cancer Center
  • Lab Reports

Imaging Screening Scheduling

To schedule your patient for any offered cancer screening, contact our team of modality-specialized Imaging schedulers at 808-691-7171.

Colon Cancer Direct Screening Program

Upon a recommendation from a referring physician, we offer direct access screening colonoscopies for patients who meet certain criteria and prefer to have a colonoscopy without a prior consultation with a GI physician.

Eligible criteria:

  • At least 50 years of age
  • In good health
  • No rectal bleeding
  • No abdominal pain
  • No change in bowel habits
