Preparing to Leave the Hospital

During your stay, your care team will work with you to plan for your discharge. You and your family members or caregivers are important members of the planning team. By planning ahead, you will have all the help and resources you need when you get home.

Make sure you know:

  • How to care for yourself after discharge. This includes activities of daily living, such as bathing, walking, meal preparation, dressing, and using the bathroom.
  • Any dietary restrictions, including what foods and beverages to avoid and which are encouraged.
  • What kinds of activities you can and can’t do, and for how long.
  • What medicines you must take, why, in what dosage and for how long.
  • What signs and symptoms to watch for that are related to your hospital stay.
  • Where you will be going after your hospital stay and how you will get there.

Please talk to your nurse and doctor about your recovery at home and any concerns you may have. We encourage you to ask questions to make sure you understand your care instructions.

Discharges are usually scheduled to occur before 11 am. 

Transportation should be arranged in advance with your family, a taxi or Handivan. If you need help, contact a member of your health care team.

All HMSA 65C Plus, HPH – Classic and Kaiser Senior Plan members are required to sign the Notification of Medicare Appeal Rights the day of discharge. If you do not receive this from your Patient Services Representative, please call 808-691-4210.

Be aware that you may need to stop at the Discharge Pharmacy and/or Patient Financial Services before leaving The Queen’s Medical Center.

Discharge Pharmacy

The Queen’s Discharge Pharmacy, located on Level 1 across from Admitting, is available to fill prescriptions at discharge. Queen’s accepts most major third party insurance policies. However, as a patient, you have the right to choose the pharmacy that will fill your prescriptions.

To streamline your discharge process, please inform a nurse of your pharmacy preference in advance. Using the Queen’s Discharge Pharmacy is convenient, but please visit your regular pharmacy for prescription refills, as they maintain a full record of your medication history. If you choose to fill your prescriptions at the Queen’s Discharge Pharmacy, be prepared to provide full payment.

Discharge Hospitality Lounge

For your comfort and convenience, Queen’s provides a Discharge Hospitality Lounge near the porté cochere where you may wait for your transportation on the day of your discharge. The lounge offers comfortable recliners, telephones, refreshments, restrooms, Wi-Fi, and television. While at the lounge, staff can assist with scheduling follow-up appointments, filling prescriptions, and reviewing discharge instructions.

Family members or caregivers may call the lounge at 691-8690 to notify the staff of the time they plan to pick you up. A member of our staff will safely escort you from the lounge to your transportation.
